Gentiva Hospice is seeking a highly skilled and compassionate RN Patient Care Manager to oversee the day-to-day operations of our hospice sites. As a key member of our team, you will be responsible for ensuring the provision of high-quality care to our patients and their families.
Key Responsibilities- Site Management: Manage the site in the absence of the Executive Director, Administrator, or Senior Patient Care Manager.
- Clinical Oversight: Oversee clinical and administrative functions, ensuring compliance with regulatory guidelines and managed care principles.
- Patient Care: Provide direct skilled bedside nursing care in patients' homes and participate in on-call nursing schedules.
- Communication: Respond to phone referrals and inquiries in a timely, professional, and compassionate manner.
- Education: Registered Nurse with a minimum of Associate's Degree in Nursing required from an accredited school of nursing.
- Experience: Three years' experience in direct patient care in a clinical setting required, with a minimum of one year of management/supervisory experience.
- Licenses and Certifications: Current license to practice nursing in the state of employment, CPR Certification, and valid driver's license.
- Sensitivity and Flexibility: Demonstrate sensitivity to the feelings of the terminally ill and their families, and flexibility in performing a variety of tasks.
- Communication and Influence: Communicate effectively with people of all socioeconomic backgrounds and influence others in their opinions and judgments.
- Leadership and Teamwork: Display evidence of patient advocacy, responsible functioning in the workplace, and maturity, cooperation, and flexibility in dealing with colleagues and the hospice team.
