About the Role:

We are seeking a skilled RN Patient Care Manager to oversee the day-to-day operations of our hospice site. As a key member of our team, you will be responsible for administering the clinical aspects of our hospice program and ensuring the provision of quality care to our patients.

Key Responsibilities:
  • Manage the site in the absence of the Executive Director or Senior Patient Care Manager.
  • Oversee clinical and administrative functions, including recordkeeping.
  • Respond to phone referrals and inquiries in a timely and professional manner.
  • Participate in on-call nursing schedules and provide direct skilled bedside nursing care in patients' homes.

Requirements:
  • Registered Nurse with a minimum of Associate's Degree in Nursing required.
  • Three years' experience in direct patient care in a clinical setting required.
  • Minimum one year of management/supervisory experience required.
  • Two or more years' experience in hospice or home care required.
  • Current license to practice nursing in the state of employment required.
