Position Overview:


The Practical Nurse Specialist is responsible for executing nursing tasks and providing patient care as directed by the healthcare facility's management team, including the Director of Nursing and/or attending Physician.

This role involves a range of responsibilities aimed at ensuring the comfort and well-being of patients requiring medical attention.

The LPN must apply specialized skills and sound judgment in delivering care, as well as implementing and assessing patient care while conducting nursing procedures to aid in the treatment and rehabilitation of patients.

Key Responsibilities:


- Deliver nursing care and perform procedures in alignment with personalized treatment plans.
- Administer prescribed medications and monitor patient responses to treatments and health status changes.
- Assist with personal care needs, including daily living activities and hygiene.
- Provide health education to patients and their families.

- Measure and document vital signs such as temperature, blood pressure, pulse, and respiration using standard methods.
- Under the guidance of a Registered Nurse, participate in the treatment of residents.

- Administer specific medications, observe and report any changes in residents' conditions, and collect specimens for laboratory analysis.

- Prepare and apply basic dressings, assist with bathing and feeding residents, and maintain a clean and safe environment by performing tasks such as making beds and changing linens.

- Ensure professional nursing services are delivered through the nursing process of assessment, planning, implementation, and evaluation/reevaluation.
- Monitor supplies to ensure they meet the needs of residents.

- Communicate patient needs effectively to the medical team for coordinated care.
- Engage in the resident admission screening process, which may involve travel as required.

- Contribute to the development and enforcement of policies and procedures.

- Participate in fiscal management, conduct performance evaluations, uphold infection control standards, manage wounds, and engage in continuous quality improvement initiatives, along with formal and informal educational activities.

- May provide oversight to subordinate professional, paraprofessional, and support staff in their resident care responsibilities as well as in facility management.


Qualifications:
- Must hold a current Licensed Practical Nurse license in New York State.
- A minimum of one year of relevant experience is required.
- BLS certification is mandatory.

Additional Information:

Shift Availability:
Primarily evenings and nights, with alternating weekends and some holidays.
