Key Responsibilities

  • Support patients in their homes, providing hands-on care and assistance with daily activities
  • Collaborate with healthcare professionals to develop and implement patient care plans
  • Communicate effectively with patients, families, and healthcare teams to ensure seamless care delivery
  • Participate in ongoing education and training to stay up-to-date on best practices and industry developments

Requirements

  • Certified Nursing Assistant Certification in the state where you'll be working
  • High School diploma or equivalent preferred
  • Hospice experience preferred
  • Current driver's license and ability to spend approximately 20% of your time driving to and from patient locations
  • The skills needed to self-manage your time and schedule
  • Demonstrated experience with tablets, mobile phones, and EMR software
