About the Role

We are seeking an experienced Advanced Practice Registered Nurse (APRN) to join our Hospice and Palliative Care team. As a key member of our interdisciplinary team, you will provide high-quality, compassionate care to patients and families in various settings, including homes, nursing homes, hospitals, and hospice care centers.

Responsibilities
  • Manage symptoms and facilitate family meetings to ensure patient-centered care
  • Assess for psychosocial, emotional, and spiritual needs, and collaborate with healthcare teams to address these concerns
  • Develop and implement individualized care plans to meet the unique needs of each patient
  • Work collaboratively with physicians, nurses, social workers, and other healthcare professionals to ensure seamless care coordination
  • Participate in ongoing education and training to stay current with best practices in hospice and palliative care
Requirements
  • Master's degree in Nursing with a focus on Advanced Practice Registered Nursing
  • Current licensure as an APRN with prescription authority in the State of Florida
  • Oncology/Hospice/Palliative Care certification preferred but not required
  • Excellent communication and interpersonal skills, with the ability to work effectively in a team environment
  • Strong analytical and problem-solving skills, with the ability to prioritize tasks and manage multiple responsibilities
