Job Summary

We are seeking a Licensed Clinical Social Worker to join our interdisciplinary team. As a key member of our team, you will work closely with patients, families, and healthcare professionals to develop and implement individualized treatment plans.

Responsibilities
  • Assess and counsel patients with medically complex needs to develop treatment plans that address adjustment to disability, resource linkage, and behavior modification.
  • Collaborate with healthcare professionals to ensure comprehensive care and support.
  • Develop and maintain relationships with community resources to provide patients with access to necessary services.
  • Conduct regular assessments and evaluations to monitor patient progress and adjust treatment plans as needed.
Qualifications
  • Master's degree in Social Work and state licensure as an LCSW are required.
  • Excellent planning, organization, writing, and time management skills are essential.
  • Ability to communicate effectively with patients, families, and healthcare professionals is crucial.
