Position Overview:
As a Service Coordinator II, you will play a vital role in providing personalized consultation, advocacy, and resource coordination for individuals with developmental disabilities requiring specialized support.

Key Responsibilities:
  • Maintain regular communication with individuals and families, developing Individual Family Service Plans (IFSP) and Individual Program Plans (IPP).
  • Engage in Interdisciplinary Team (IDT) meetings and collaborate with community partners, including educational institutions and day programs.

Facilitate access to community resources, including:
  • Local schools, recreational activities, and support groups.
  • Employment and habilitation programs.
  • Agencies such as C.C.S., Department of Rehabilitation, and Medi-Cal.
  • Specialist services, including speech and occupational therapy.

Service Coordination:
Ensure the delivery of TCRC-funded services, which may include:
  • Behavioral services and independent living training.
  • Respite care and out-of-home placements.
  • Day programs for infants and adults.
  • Durable medical equipment and specialized services.

Essential Functions:
  • Conduct assessments to understand the unique challenges faced by families.
  • Involve individuals and families in person-centered planning and support.
  • Design, document, and monitor service plans effectively.
  • Facilitate team progress and identify necessary outcomes and supports.
  • Build and maintain relationships with community vendors and agencies.
  • Provide training and information on developmental disabilities and available resources.
  • Utilize strong communication skills to respect and support individuals with developmental disabilities and their families.
  • Demonstrate knowledge of community resources and integrate them into service planning.
  • Participate in ongoing professional development and training opportunities.
  • Conduct all activities with professionalism and ethical standards.

Qualifications:
  • Bachelor's degree in Human Services or a related field.
  • Minimum of 2 years of experience in social or mental health services.
  • Valid California driver's license and reliable transportation.
  • Experience with individuals with developmental disabilities is preferred.

Physical Requirements:
Ability to operate standard office equipment, maintain focus on tasks for extended periods, and travel to various locations as needed. Physical tasks may include lifting up to 20 lbs and navigating uneven terrain.
