Position Overview

The main objective of this role is to deliver proficient nursing care to residents under the guidance of their attending physician, adhering to the nursing practice standards established by the state.

Key Responsibilities

Administrative Duties
  • Supervise Certified Nursing Assistants (CNAs) and other nursing staff as directed by the Nursing Director.
  • Collaborate with the interdisciplinary team (IDT) and healthcare professionals to evaluate, plan, implement, and assess personalized care plans for residents.
  • Ensure timely completion of initial and ongoing comprehensive assessments and care plans.
  • Assist in the completion of sections of the Minimum Data Set (MDS) assessment as needed.
  • Prepare for physician rounds by organizing charts, highlighting concerns, and preparing orders for physician signatures; document physician visits with residents.
  • Initiate referrals or consultations as required.
  • Report any suspected criminal activity involving residents.
  • Complete administrative tasks including medical forms, reports, evaluations, and charting.
Committee Involvement
  • Engage in quality committees such as quality assurance and performance improvement (QAPI), infection control, and risk management.
  • Participate in various facility committees as appointed by the Administrator.
Resident Care Responsibilities
  • Deliver nursing services to residents in accordance with established policies and professional standards.
  • Monitor residents for acute changes in condition, including confusion, fever, or pain, and conduct assessments as necessary.
Additional Duties
  • Track chronic health conditions of residents and be aware of significant changes.
  • Assist with the admission process and provide orientation to new residents.
  • Support discharge planning focused on residents' goals and transitions to post-discharge care.
  • Ensure newly admitted residents have immediate physician orders for care.
  • Participate in various assessments using appropriate MDS forms.
  • Contribute to the creation of individualized, resident-centered care plans.
  • Provide compassionate nursing care, especially for residents with cognitive decline or trauma history.
  • Monitor residents' nutritional intake and report significant changes.
  • Maintain thorough documentation of all nursing care provided.
  • Collaborate with hospice services for end-of-life care.
  • Promote safety practices to prevent falls.
  • Administer first aid as needed and follow infection control protocols.
  • Administer medications per physician orders and report any adverse effects.
  • Transcribe orders from providers as appropriate.
  • Oversee CNAs as directed.
  • Assess and manage resident pain in accordance with provider orders.
  • Assist residents with personal care and daily living activities.
  • Help reposition bed-bound or chair-bound residents as scheduled.
  • Monitor skin health and provide necessary treatments.
  • Collect specimens for lab analysis as ordered.
Personnel Functions
  • Work collaboratively with other staff to achieve departmental goals and maintain positive relations.
  • Assist in the training and development of new nursing team members.
Professional Development
  • Participate in continuing education and training programs to enhance care quality.
  • Assist in planning and conducting in-service training sessions as requested.
Safety and Sanitation
  • Ensure a safe environment for residents by minimizing accident hazards.
  • Provide adequate supervision and assistive devices to prevent accidents.
Equipment Use

Utilize personal protective equipment (PPE) as necessary, including masks, gowns, and gloves.

Working Environment
  • Work in various areas, including offices and nursing service areas.
  • Experience frequent interruptions and engage with residents, families, and staff under diverse conditions.
  • Be available for on-call duties as needed.
  • Participate in community health initiatives as appropriate.
Qualifications

Must possess a nursing degree from an accredited institution or be a graduate of an approved LPN/RN program.

Experience

A minimum of two years of experience as an LPN/RN is required.

Specific Requirements
  • Must hold a current, unencumbered nursing license.
  • Ability to communicate effectively in English.
  • Demonstrate knowledge and skills to provide age-appropriate care.
  • Exhibit teamwork and leadership qualities.
  • Ability to make independent decisions when necessary.
  • Must be tactful in interactions with residents and families.
  • Knowledge of nursing practices and regulations in long-term care.
  • Must be flexible and able to work effectively with diverse populations.
Physical and Sensory Requirements
  • Ability to move throughout the workday and communicate effectively.
  • Ability to cope with the mental and emotional demands of the position.
  • Must meet health requirements as per facility policies.
  • Ability to assist in emergencies as needed.
