Transitions Program CoordinatorDepartment: State Operations & Programs

Role Summary:The Transitions Program Coordinator will undertake a variety of responsibilities aimed at teaching, counseling, assisting, and supporting individuals to enhance their independence through skill development and pre-vocational training. This role will employ a person-centered approach to deliver support and case management services for individuals, effectively engaging with their families, employers, and other community providers. The Coordinator will facilitate educational and vocational groups to assist individuals in developing social, vocational, and workplace competency skills.
Qualifications:- Experience in job coaching, counseling, or special education; or a Bachelor's Degree in a related field from an accredited institution along with one year of relevant experience.
- Preferred experience as an employment consultant with a proven track record in employer communications and relationship management.
- Ability to adapt and quickly develop and balance multiple relationships to achieve results.
- Strong skills in presentation, facilitation, project management, and time management.
- Excellent written communication skills with a keen attention to detail; demonstrates initiative, a drive for results, and self-assessment capabilities, along with the ability to work independently and collaboratively.
- Comfortable engaging with diverse individuals, meeting new people, and addressing sensitive topics.
- Willingness to travel locally, utilize a personal cell phone, and work evenings and weekends as necessary to fulfill job responsibilities.
- Access to a vehicle with appropriate insurance coverage.
- Employment is contingent upon accreditation by the governing state agency.
- Secure program partners and participants, including school districts and employers, while meeting established benchmarks for partner and participant retention.
- Collaborate with Friendship Program staff to identify potential participants and school district partners.
- Work with Jobs Program staff to create work-based learning experiences and opportunities for permanent job placements following program completion.
- Establish positive relationships with families, support coordinators, referral sources, and other community stakeholders.
- Coordinate and implement program curriculum through instruction, small group activities, and one-on-one support as needed.
- Assess employer needs and collaborate to identify valuable internship positions that align with program objectives.
- Provide job coaching during the work-based learning experience phase.
- Manage a caseload of program participants effectively.
- Develop and refine program curriculum as necessary.
- Assist supervisors with fundraising events as directed.
- Collaborate with the State Director and/or supervisor to enhance awareness of the organization.
- Engage program participants in various activities.
- Complete required documentation in a timely and organized manner.
- Maintain an organized filing system for all relevant documentation.
- Update databases with all services provided to participants and employer communications.
- Work with supervisors to ensure accurate and timely completion of all required billing.
