Position Overview

The Patient Flow Coordinator serves a pivotal role within the Clinical Command Center, focusing on optimizing patient throughput and bed management across the healthcare system.

Key Responsibilities
  • Leadership: Act as the team lead for the Patient Flow Coordinator team, overseeing bed management queues, documentation, and staff assignments.
  • Operational Support: Provide guidance to patient flow staff during challenging situations, ensuring efficient placement of emergent patients and managing bed placement workflows.
  • Education: Facilitate training and re-education of team members on new workflows and procedures.
  • Issue Resolution: Address and escalate issues to the appropriate managerial staff when necessary.
  • Collaboration: Work closely with various departments, including Transfer Center and Behavioral Health, to ensure timely patient placements.
Patient Coordination

The Patient Flow Coordinator is responsible for:

  • Strategizing patient placements based on acuity levels and hospital capacity.
  • Managing admissions, transfers, and placements in alignment with organizational policies.
  • Utilizing real-time capacity planning tools to facilitate patient movement.
Performance Monitoring

Monitor bed planning performance and engage in problem-solving to maintain positive patient flow. Collaborate with Environmental Services to prioritize room cleaning and prepare for daily bed meetings.

Reporting and Compliance

Prepare end-of-shift reports and maintain a professional image while demonstrating excellent customer service. Ensure compliance with regulatory requirements and quality initiatives.

Qualifications

A Bachelor's Degree in a relevant field is required, along with a valid RN or LPN license and a minimum of five years of experience in a clinical environment.
