Role Responsibilities

As an Inpatient Care Coordinator RN, you will be part of a clinically adept and collaborative team, thriving in an environment that promotes innovative thinking and shared learning. Your responsibilities will include:

  • Conducting initial and ongoing utilization reviews, discharge planning, and care coordination, utilizing evidence-based criteria alongside your clinical expertise to ensure appropriate admissions and timely discharges.
  • Monitoring and evaluating the severity of illness and treatment responses, making informed recommendations for continued care or transitions to lower levels of care.
  • Employing critical thinking and problem-solving skills to address complex clinical situations effectively.

Qualifications and Skills

  • Strong understanding of utilization management standards and medical necessity criteria, with the ability to extract and analyze relevant clinical data.
  • Excellent communication skills, both verbal and written, to advocate for optimal patient experiences and outcomes.
  • Ability to build collaborative relationships with healthcare providers, fostering a shared vision for patient care.
  • Bachelor of Science in Nursing is preferred.
  • Current, unrestricted RN licensure in California is required.
  • A minimum of 3-5 years of recent clinical experience in an acute care setting.
  • Proficiency in computer skills, including EMR and Microsoft Office Suite.
  • Experience in managed care concepts is desirable.
  • Previous discharge planning experience is a plus.
