Patient Advocate Manager

Pinnacle Treatment Centers OH - IX, LLC is committed to delivering exceptional patient care and advocacy. As a Patient Advocate Manager, you will be responsible for coordinating patient services, providing emotional support, and empowering patients to make informed decisions about their care.
Key Responsibilities:
- Provide empathetic and non-judgmental support to patients and their families, addressing concerns and answering questions about treatment and recovery.
- Facilitate communication between patients, healthcare providers, and other stakeholders to ensure seamless care coordination.
- Develop and implement patient-centered care plans that address physical, emotional, and social needs.
- Collaborate with treatment teams to provide comprehensive and coordinated care.
- Conduct regular outreach to patients, assessing their evolving needs and adjusting care plans accordingly.
Requirements:
- Bachelor's degree in social work, psychology, or a related field required; master's degree preferred.
- 3+ years of experience in patient advocacy, social work, or a related field; experience in addiction treatment an asset.
- Strong understanding of healthcare systems, community resources, and addiction treatment modalities.
- Excellent communication and interpersonal skills, with ability to work effectively with diverse populations.
- Ability to maintain confidentiality and handle sensitive information discreetly.
Salary and Benefits:
- Competitive salary range: $70,000 - $90,000 per year, depending on experience.
