Job Overview

We are seeking a dedicated and compassionate Mental Health Case Manager to join our team at Zepf Center. As a key member of our community-based care team, you will play a vital role in providing support and coordination services to individuals coping with mental illness.

About the Role

The successful candidate will be responsible for assisting clients with the development of short and long-term goals, acquiring community resources, and ensuring their needs are met. You will work collaboratively with clients, families, and other stakeholders to create comprehensive care plans, advocate for individuals, and facilitate assessments, linkages, and referrals.

Your expertise will be utilized to evaluate client strengths and requirements, track progress towards achieving goals, and reassess and adjust plans as needed. Additionally, you will provide active outreach, crisis intervention and management, and support clients in achieving outcomes.

Key Responsibilities
  • Evaluate client strengths and requirements, collaborating with clients and relevant team members to formulate detailed care coordination and treatment strategies.
  • Maintain contact with clients as indicated in the treatment plan or as needed to ensure stability, communicating with and providing support to achieve goals and outcomes.
  • Link and/or refer clients to other services, programs, and agencies as needed, assisting in ensuring clients obtain and retain applicable entitlements.
  • Serve as an advocate for clients, communicating with family members and significant others as indicated with appropriate releases.
Requirements and Qualifications
  1. High School Diploma or GED required with demonstrated mental health experience; or Bachelor's degree in social work or three-years of mental health experience in behavioral health - preferred.
  2. CDCA certification required for SUD.
  3. Must have valid driver's license and be able to provide proof of auto insurance.
Benefits and Working Conditions

This position offers a range of benefits, including a competitive salary, generous time off, tuition reimbursement, and a 403(b) retirement plan with employer match. The ideal candidate will possess a valid driver's license, acceptable driving record, and be willing to work in offices, participant's homes, and other community sites. Salary: $45,000 - $60,000 per annum, depending on experience.
