Post-Acute Long-Term Care Nurse Navigator Specialist

At St. Peter's Health, we are seeking a highly skilled and entrepreneurial Nurse Navigator to lead our skilled nursing facility patient care team.
The successful candidate will collaborate with providers and clinical areas to coordinate services and ensure quality of care for patients.
Key responsibilities include assessing the physical and psychosocial needs of patients and their families, prioritizing workload, and responding to facility calls and messages in a timely manner.
The ideal candidate will possess strong communication and time management skills, as well as the ability to perform research on clinical protocols and provide empathetic supportive counseling to patients with life-threatening disease.
As a member of our multi-disciplinary care team, the Nurse Navigator will work to improve patient outcomes and provide services with the goal of improving the patient's experience.
Aptitudes:
- Effective communication skills, both oral and written
- Effective time management skills, ability to effectively complete multiple tasks accurately and timely
- Ability to perform research on clinical protocols and to assist physicians to develop and maintain protocols
- Ability to provide empathetic supportive counseling to patients with life-threatening disease
- Understanding of providing services with the goal of improving the patient's experience
- Ability to work effectively as a member of a multi-disciplinary care team to improve patient outcomes
