Job Title: Senior Hospice Care Nurse Specialist

We are seeking an experienced and compassionate Senior Hospice Care Nurse Specialist to join our team at Liberty Health.

About the Role:

This is a full-time opportunity for a dedicated LPN (Licensed Practical Nurse) to provide exceptional patient care, working closely with our registered nurses, patient care coordinators, and other agency personnel.

Key Responsibilities:
  • Deliver direct patient care, utilizing various skills and coordination of services, while ensuring quality care and maintaining a safe environment.
  • Accurately document patient information, following policy, procedure, and regulation guidelines.
  • Collaborate with the registered nurse to develop and implement effective care plans tailored to each patient's diagnosis and condition.
  • Coordinate hospice services, interacting with patients, families, community agencies, physicians, and other healthcare professionals.
  • Receive and transcribe physician orders as directed.
  • Ensure timely completion and submission of clinical, progress, and summary notes to facilitate seamless care coordination.
  • Participate in interdisciplinary team meetings to optimize patient care outcomes.
Requirements:
  • A minimum of one year of clinical nursing experience, preferably in acute care medical/surgical settings.
  • Current LPN licensure in the state where the branch is located.
  • Ability to work independently, prioritizing tasks, and providing high-quality patient care services.
  • Excellent communication and documentation skills.
  • CPR certification.
Compensation and Benefits:
