Salary: $60,000 - $80,000 per year, depending on experience.

Job Summary: We are seeking a highly skilled Counselor Specialist to join our team. As a key member of our interdisciplinary team, you will be responsible for providing high-quality psychosocial services to patients and their families.

Key Responsibilities:

  • Patient Assessments: Conduct comprehensive initial and ongoing assessments of patients' social, emotional, and environmental needs.
  • Counseling and Support: Provide effective counseling approaches and processes to patients and their families, including therapeutic interventions, education, and life review.
  • Care Planning: Develop and implement personalized plans of care in collaboration with the interdisciplinary team.
  • Communication: Communicate effectively with patients, families, and healthcare professionals to ensure seamless care transitions.
  • Documentation: Maintain accurate and timely documentation of patient interactions and progress.

Requirements:

  • Education: Bachelor's degree in Social Work (BSW) or Master's degree in Social Work (MSW) from a Council on Social Work Education (CSWE) accredited program.
  • Licensure: Current Social Work licensure in Ohio or Indiana, with eligibility to obtain licensure in the other state.
  • Experience: Minimum of two years of experience in a clinical setting, preferably in healthcare or hospice.
  • Skills: Strong assessment, counseling, and communication skills, with ability to work effectively in a fast-paced environment.
