About the Role

We are seeking a highly skilled Care Navigator to join our team at Elderwood IPA in Buffalo, NY. As a Care Navigator, you will play a vital role in supporting our members and ensuring they receive the best possible care.

Key Responsibilities
  • Assist with outreach to medical providers to obtain necessary documentation or schedule appointments.
  • Answer incoming calls and provide support to members, routing calls to care managers as needed.
  • Complete call audits to ensure quality and accuracy.
  • Coordinate and manage incontinence supply requests.
  • Upload documents into our Electronic Medical Record (EMR) system as needed and assist with mailings.
  • Monitor member satisfaction through phone calls, answering questions, and providing information.
  • Document member service issues, identify trends, and recommend potential solutions.
  • Participate in interdisciplinary team meetings and provide input on customer service-related activities.
  • Work with the care team to support timely communication of member issues or needs and monitor screening of members effectively to improve quality and cost outcomes.
  • Understand and comply with our quality management program and care coordinators' role within that program.
  • Maintain member confidentiality and adhere to our policies and procedures.
  • Accurately document phone conversations in our designated PHI system.
  • Embrace change and maintain an open mind, being flexible and adaptable in the face of ambiguity and change.
Requirements
  • Associate degree or equivalent work experience.
  • Demonstrated knowledge of Medicaid product information.
  • Basic understanding of health diagnoses and verbiage.
  • Proficiency with Microsoft Office.
  • Current, valid NYS driver's license.
  • Experience in the insurance field is preferred.
  • Experience working with geriatric or Medicaid populations is preferred.
  • Bilingual English/Spanish speaking is preferred.
