Primary Care Nurse

We are seeking a skilled and compassionate Primary Care Nurse to join our team at ChristianaCare Direct in Elkton, MD.
About the Role:
- Provide high-quality, patient-centered care to individuals of all ages, from pediatric to geriatric.
- Collaborate with healthcare providers to develop and implement individualized care plans.
- Conduct home visits to assess patients' needs and provide education on self-management of chronic conditions.
- Participate in a relationship-based care approach to deliver care that is tailored to each patient's unique needs.
- Perform point-of-care tests, EKGs, and other tests as directed by clinicians.
- Triage patients and ensure appropriate disposition if on-site.
- Respond to patient crises situations and provide emotional support.
- Update medication histories and assist with staff education and evaluation of clinical competencies.
- Support the site by acting as a liaison for JCAHO, POCT, and Tracer monitor for Safe Practice Behavior Monitoring (SPBM) tools.
Requirements:
- A graduate of a state-approved or National League of Nursing accredited School of Nursing.
- RN license or eligible for licensure in the state of Maryland.
- Two years' experience as a RN, physician office experience preferred.
- BSN preferred; ADN considered with agreement of completion of BSN in 3 years.
Special Requirements:
- BLS certification required.
Proof of a valid driver's license and active auto insurance is required.
