We are seeking a skilled and compassionate Registered Nurse to join our Primary Care team in Elkton, MD. As a key member of our team, you will provide high-quality, patient-centered care to individuals in their homes, promoting optimal health outcomes and improving quality of life.
Key Responsibilities- Deliver evidence-based nursing care to patients in their homes, including assessments, education, and support.
- Collaborate with healthcare providers to develop and implement individualized care plans.
- Conduct home visits to assess patients' needs and provide education on self-management of chronic conditions.
- Perform point-of-care testing, EKGs, and other tests as directed by clinicians.
- Triage patient calls and respond to messages under the direction of the provider.
- Assist providers with examinations and procedures as needed.
- Communicate effectively with patients, families, and healthcare teams to ensure seamless care coordination.
- Participate in quality improvement initiatives to enhance patient outcomes and satisfaction.
- Graduate of a state-approved or National League of Nursing accredited School of Nursing.
- RN license or eligibility for licensure in the state of Maryland.
- Two years of experience as a RN, with physician office experience preferred.
- BSN preferred; ADN considered with agreement of completion of BSN in 3 years.
- BLS certification.
- Valid driver's license and active auto insurance.
