CenterWell Primary Care Anywhere is seeking a highly skilled and compassionate Nurse Practitioner to join our team of healthcare professionals. As a key member of our organization, you will play a vital role in delivering high-quality, patient-centered care to our community.
Job SummaryWe are looking for a dedicated and experienced Nurse Practitioner to provide primary care services to our patients. The ideal candidate will have a strong background in geriatrics, internal medicine, or primary care, and will be passionate about delivering exceptional patient care in a house call setting.
Responsibilities- Provide comprehensive primary care services to patients in their homes, including health promotion, disease prevention, and management of acute and chronic conditions.
- Conduct thorough physical exams, take medical histories, and order diagnostic tests as needed.
- Develop and implement individualized treatment plans, including medication management and lifestyle modifications.
- Communicate effectively with patients, families, and healthcare teams to ensure seamless care coordination.
- Stay up-to-date with the latest medical research, guidelines, and best practices in primary care.
- Master's degree in nursing or related field.
- Minimum 2 years of experience as a Nurse Practitioner in a primary care setting.
- Active, unrestricted state license and DEA registration.
- Board certification in a relevant specialty (e.g., gerontology, internal medicine).
- Strong technical skills, including proficiency in electronic medical records (EMRs) and other healthcare software.
- Excellent communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare teams.
- House call experience or experience working in a similar ambulatory care setting.
- Urgent care, primary care, or internal medicine experience.
