Job Summary

We are seeking a skilled and compassionate Licensed Practical Nurse (LPN) to join our team at Hubbard County Heritage Living Center. As an LPN, you will play a vital role in providing high-quality patient care and contributing to the overall well-being of our residents.

Key Responsibilities
  • Provide nursing care to residents/patients within the scope of practice of a Licensed Practical Nurse (LPN).
  • Coordinate and direct resident/patient care, ensuring seamless transitions and effective communication with other departments/providers.
  • Assist Registered Nurses with completion of assessments, documentation, and data collection, acting timely on findings.
  • Administer medications and complete treatments as ordered, observing and monitoring resident's condition and reporting changes as appropriate.
  • Maintain a clean, sanitary, safe, and efficiently organized work area, adhering to infection control policies and procedures.
Requirements
  • Degree from an accredited school of nursing for the Licensed Practical Nurse program.
  • Current LPN licensure in the state in which you are working.
  • Up to 2 years' experience as LPN.
  • Ability to communicate effectively in both written and verbal formats.
  • Basic computer skills.
  • Ability to follow all safety rules, regulations, policies, and procedures of the facility.
