We are seeking a skilled Wound Clinician to join our team at Yale New Haven Health. As a key member of our interdisciplinary care team, you will provide expert care to patients with wounds, ostomies, and incontinence. Your clinical expertise and leadership skills will enable you to consult with staff, physicians, and other healthcare professionals to ensure the highest quality patient care.
Responsibilities
• Provide direct and indirect care to patients with wounds, ostomies, and incontinence
• Serve as a clinical resource and consultant for staff, physicians, and members of the interdisciplinary care team
• Evaluate patient response to treatment and the progress of wound healing, making adjustments and modifications as indicated
• Provide education to patients, caregivers, and staff regarding skin care, wound management, and ostomy management
• Assist staff in maintaining current state-of-the-art practice by reviewing and revising policies and procedures to be consistent with national guidelines and other sources of evidence-based literature
• Support unit leadership in ensuring that customer service standards reflecting excellence consistent with Hospital policy are met
Qualifications
• BSN required; Masters in Nursing preferred
• Minimum of 2-3 years clinical experience with wound management in an acute care setting
• WOC certification from WOCNCB or evidence of enrollment
• Excellent assessment skills and understanding of factors impacting wound healing outcomes
Work Environment
This position will work on both the Bridgeport and the Milford campuses. The employee is required to use hands to finger, handle, or feel; reach with hands and arms; and talk or hear. The employee is required to stand and walk. The employee must frequently stoop, kneel, crouch, or crawl; talk or hear. The employee must regularly lift and/or move up to 25 pounds, occasionally move and/or lift up to 50 pounds. The specific vision abilities required include close vision, distance vision, color vision, peripheral vision, depth perception, and ability to adjust focus. The noise level in the work environment is usually moderate.
