We are seeking a highly skilled Pharmacy Care Coordinator to join our team at Empath Health. As a Pharmacy Care Coordinator, you will play a critical role in ensuring the smooth operation of our pharmacy services, working closely with our pharmacy team and other stakeholders to provide exceptional patient care.
Key Responsibilities:- Assist pharmacists with daily pharmacy operations, including coordinating 340B pharmacy services and ensuring compliance with all aspects of the 340B pricing program.
- Manage incoming calls and faxes, handling caller inquiries with a pleasant and friendly demeanor.
- Complete data entry of patient and prescription information into our pharmacy system.
- Coordinate patient welcome and refill compliance calls, as well as manage communication with prescribers to resolve issues and provide updates on patient and prescription status.
- Adjudicate prescription claims to ensure accurate billing and payment for products.
- Investigate benefits and resolve payer rejections, including prior authorization coordination and appeals.
- Enroll and coordinate patient financial assistance and support services, such as copay cards and funding assistance programs.
- Work with patients to synchronize medication dispenses and assist with adherence programs.
- Coordinate prescriptions for pick up or home delivery, tracking all deliveries and ensuring signature paperwork is filed appropriately.
- Maintain compliance with company policies and procedures, as well as all laws regulating pharmacy services.
- High school diploma or general education degree (GED); or one to three months related experience and/or training; or equivalent combination of education and experience.
- Florida Pharmacy Technician License.
- Continuing education: 20 Hours Biennial.
- Knowledge of Pharmacy Benefit Management (PBM) procedures and processes.
- Ability to compile and analyze data from various sources.
- Strong analytical and problem-solving skills.
- Intermediate Excel and report skills.
- Excellent communication and organization skills.
- Good knowledge of medical and pharmaceutical terms and abbreviations.
- Skill in typing/keyboarding at a minimum of 30 words per minute.
- Minimum 3-5 years of experience in Retail, Managed care, PBM, Hospital, Home Health Care, or Long Term Care Pharmacy settings desired.
