About the Role

Amerita is a leading provider of specialty infusion services, focused on delivering complex pharmaceutical products and clinical services to patients outside of the hospital. We are seeking a highly skilled Clinical Pharmacist to join our team in one of our pharmacies located in Oklahoma City, Denver, Atlanta, Fairfield (NJ), or Fort Myers (FL).

Key Responsibilities
  • Evaluate physician medication orders for appropriateness, ensuring optimal medication management for patients.
  • Provide support for infusion pharmacy dispensing and fulfillment operations, engaging in business improvement and sustainability initiatives.
  • Make recommendations for therapy changes to physicians and facility staff, ensuring the best possible outcomes for patients.
  • Continuously review products selected for each patient to validate margin, formulary changes, and medication appropriateness based on disease state.
  • Make decisions to approve or deny claims, ensuring accurate reimbursement and minimizing losses.
  • Audit previous approvals to ensure losses are appropriate and reimbursement is up to date.
  • Identify brand to generic conversion opportunities to enhance probability and minimize copay costs to patients.
  • Serve as a lead resource for internal stakeholders on medical/drug information inquiries.
  • Identify problem areas and coordinate activities with cross-functional team members to address root causes.
  • Maintain patient databases, including current and past medical history, clinical and laboratory data, and concurrent medication.
  • Collaborate with the healthcare team to provide clinical and technical support as needed.
  • Document all communication with physicians, nurses, and other healthcare team members in the medical record.
  • Respond to patient-specific or non-patient-specific inquiries from internal or external stakeholders for drug/medical information.
  • Document adverse events and product complaints per contractual obligations to pharmaceutical manufacturers.
  • Assist facility/agency staff personnel with procedures relating to safe and effective methods of procuring, maintaining, administering, and disposing of drugs.
Requirements
  • Bachelor's Degree in Pharmacy or PharmD.
  • One or more years of specialty or infusion pharmacy practice experience.
  • Knowledge of pharmacy laws and federal and state regulations concerning pharmaceutical care practices for infusion patients in home and at an infusion facility.
  • Ability to work cooperatively as a member of a team.
  • Strong problem-solving skills.
