Job Title: Clinical Pharmacist

Option Care Health is seeking a highly skilled Clinical Pharmacist to join our team. As a Clinical Pharmacist, you will play a critical role in providing safe and effective pharmacy services to our patients in the comfort of their own homes.

Key Responsibilities:
  • Design and implement patient-specific pharmacy care plans for home care patients
  • Assess patients' suitability for home care services and make recommendations for modifications in prescribed medication therapy
  • Provide medication-use education to patients, caregivers, and healthcare professionals
  • Ensure continuity of pharmaceutical care to and from the home and other patient-care settings
  • Comply with state and federal laws, as well as USP standards, including DEA controlled substance regulations
Requirements:
  • Graduate of an accredited School of Pharmacy
  • Current, active Pharmacy license as granted by the State Board of Pharmacy
  • At least one year of experience in sterile IV admixture procedures, or knowledge of non-sterile compounding, immunizations, and medication therapy management
Preferred Qualifications:
  • Completion of an ASHP accredited Pharmacy Residency Program or an Advanced Pharmacy Practice Experience associated with an accredited school of pharmacy
  • Experience in home care, hospital, or other clean room setting
