Job Description
We are seeking a skilled Licensed Clinical Social Worker to join our team. As a key member of our interdisciplinary care team, you will work closely with patients, families, and healthcare providers to develop and implement individualized care plans.
Responsibilities
- Assess patient needs and develop comprehensive care plans
- Educate patients and caregivers on disease management and treatment options
- Collaborate with healthcare teams to ensure seamless care coordination
- Provide emotional support and counseling to patients and their families
Required Skills and Qualifications
- Master's degree in Social Work (MSW) from an accredited institution
- Current state licensure as a Licensed Clinical Social Worker (LCSW)
- Minimum 2 years of clinical experience in an ESRD setting
- Excellent communication and interpersonal skills
