Job Title: Care Coordinator - Nursing Care

Job Summary:

The Care Coordinator - Nursing Care is responsible for providing leadership and direction to nursing staff as they provide care and service. This role involves coordinating the assessment and care planning process to ensure that each resident achieves or maintains the highest level of functioning.

Key Responsibilities:
  • Provide leadership to the nursing team in a professional and positive manner.
  • Work closely with nurses, nursing assistants, medication assistants, and unit clerks to establish a positive working environment.
  • Communicate concerns and/or problems with the DON in a timely manner, proposing possible solutions or remedies.
  • Manage the day-to-day functions of nursing activities in accordance with current regulations, best practice nursing standards, and Pleasant View guidelines.
  • Assist in developing methods for coordinating nursing services with other resident services to ensure continuity of care.
  • Perform duties such as completion of the Minimum Data Set (MDS), coordination of the assessment and care planning process, and coordination of physician rounds.
  • Provide requested reports for the facility's quality assurance program and submit them to the DON in a timely manner.
  • Participate in surveys (inspections) made by authorized government agencies.
  • Make written and oral reports and recommendations to the DON as necessary and required, concerning the operation of the Nursing Department.
  • Coordinate the admission, transfer, and discharge process with the resident and family in conjunction with the Clinical Assessment Nurse and the Social Worker.
  • Complete accident/incident and skin injury reports as necessary, reviewing them for completeness and completing the investigation of the root cause.
  • Monitor the follow-up review of the care plan for effective and appropriate interventions.
  • Attend various committee meetings of the facility as required.
  • Participate in developing the agenda for Care Plan committee meetings as necessary.
  • Participate in regularly scheduled Medicare meetings, reporting on the goals and progress toward the IDT goals.
  • Monitor the Medicare documentation for completeness in supporting the care and services that are being provided under skilled care nursing.
  • Communicate with Medical Providers, reviewing the plan of care with updates as to improvements, changes in condition, and decline in condition.
  • Report and investigate all allegations of resident abuse, neglect, misappropriation of property, resident-to-resident altercation, or injuries of unknown origin.
  • As assigned, submit the online report to BQA and complete the final report for submission to BQA.
  • Share weekend on-call rotation with other nursing management staff.
  • Complete other duties as assigned by the DON.
Personnel Functions:
  • Assist the DON in the development of nursing department goals and plans.
  • Actively support the goals and steps to achieve the goals in a collaborative manner.
  • Make daily rounds, observing that nursing service personnel are performing their work assignments in accordance with acceptable nursing standards.
  • Present findings in a timely manner to the employee, providing necessary education and discipline as necessary.
  • Report these findings and the action taken to the DON.
  • Assist as needed with the coordination of the nursing schedule in the event that the scheduler is not available.
  • Review complaints and grievances made or filed by department personnel.
  • Make appropriate reports to the DON as required or as may be necessary.
  • Present disciplinary action in a private setting and administer the action in a professional manner.
  • Establish a good understanding with the employee regarding expectations.
  • Develop and maintain a good working rapport with personnel within the nursing department and other departments within the facility.
  • Create and maintain an atmosphere of warmth, personal interest, and positive emphasis, as well as a calm environment throughout the unit and shift.
  • Meet with the team on a regularly scheduled basis to promote effective communication, timely problem-solving, and the improvement of services.
  • Attend all mandatory meetings and in-services and complete the make-up session in a timely manner.
  • Encourage night shift attendance as well.
  • Monitor that privacy and confidentiality of resident information are upheld.
  • Monitor that facility computers are used for resident-related services only.
  • Monitor that employee cell phones are kept in the employee locker room or employee vehicle.
Nursing Care Functions:
  • Inform nursing service personnel of new admissions and information about the new resident.
  • Audit the appearance and staging of the room so that it is ready when the resident arrives.
  • Greet the new resident upon admission.
  • Review nurses' documentation, monitoring that it is descriptive of the nursing care being provided and reflective of the resident's response to care.
  • Make rounds with physicians as necessary.
  • Encourage attending physicians to record and sign progress notes, physicians' orders, etc., on a timely basis.
  • Make daily resident visits to observe and evaluate the resident's physical and emotional status.
  • Monitor medication administration and treatment schedules, observing that they are being administered as ordered and provided as scheduled.
  • Review medication records for completeness of information, accuracy in the transcription of physician orders, and adherence to stop order guidelines.
  • Update the DON and the Administrator of the status of residents as necessary.
  • Review the plan of care with residents, and/or family members, as necessary.
  • Openly discuss concerns or challenges with the intent to find an agreeable plan.
  • Inspect the nursing service treatment areas daily to monitor that they are maintained in a clean and safe manner.
  • Consult with the resident's physician in planning resident care, treatment, rehabilitation, etc., as necessary.
  • Notify the resident's attending physician and HCPOA when there is a significant change in the resident's condition.
  • Start IVs, obtain sputum, urine, and other lab tests as ordered.
  • Admit, transfer, and discharge residents as necessary.
  • Assist the Unit Nurse in monitoring seriously ill residents.
  • Inform family members of the death of a resident as necessary.
Safety and Sanitation:
  • Monitor your shift's personnel, observing that they are following established safety regulations in the use of equipment and supplies.
  • Monitor that nursing service work areas are maintained in a clean and sanitary manner.
  • Monitor that all resident rooms, treatment areas, etc., are maintained in a clean, safe, and sanitary manner.
  • Monitor that nursing service personnel participate in fire safety and disaster preparedness drills in a safe and professional manner.
  • Monitor that nursing service personnel follow infection control guidelines.
  • Monitor that nursing service personnel follow established hand-washing guidelines.
Care Plan and Assessment Functions:
  • Participate in the development of written preliminary and comprehensive assessments of the nursing needs of each resident as necessary.
  • Participate in the development of a written plan of care (preliminary and comprehensive) for each resident that identifies the problems/needs of the resident, indicates the care to be given, goals to be accomplished, and which professional service is responsible for each element of care.
  • Insofar as practical, encourage the resident to participate in the development and review of his/her plan of care.
  • Review resident care plans for appropriate resident goals, problems, approaches, and revisions based on nursing needs.
  • Observe that personnel involved in providing care to the resident is aware of the care plan and that care plans are used in administering daily care to the resident.
  • Complete regular audits of the process for managing resident change of condition, observing that the process is complete and completing aspects of the process that have not been completed.
  • Prepare the respective written report on the audit tool.
Resident Rights:
  • Maintain the confidentiality of all resident personal health information.
  • Monitor that residents are treated fairly, and with kindness, dignity, and respect.
  • Monitor that nursing care is provided in privacy and that nursing service personnel knock before entering the resident's room.
  • Monitor that nursing service personnel are knowledgeable of the residents' rights.

Qualifications:

  • Minimum of two (2) years' experience as a registered nurse with a current unrestricted Wisconsin license.
  • At least one (1) year experience in long-term care and/or acute hospital care.
  • Minimum of one (1) year nursing management experience.

  • Ability to use advisory and design data and information such as technical manuals, resident records, admission and discharge documents, physician's orders and reports, budget reports and summaries, nursing staff certification and licenses, payroll records, lab reports, drug reference manuals, incident/accident reports, medical charts, Bureau of Quality Assurance memos and audit reports, rules and regulations regarding long-term care, billing statements, resident care plans, restraint consent forms, nursing guidelines, infection control manuals, personnel guidelines, non-routine correspondence, professional nursing journals and periodicals, laws, and educational curricula.
  • Ability to communicate effectively with nursing personnel, PV department supervisors, therapists, physicians, County Supervisors, nursing educators, hospital and clinic personnel, residents, residents' families, lab technicians, pharmacists, clergy, insurance company representatives, vendor representatives, Administrator, State and Federal health care regulators.
  • Mathematical Ability: Ability to calculate percentages, fractions, and decimals; ability to calculate volume, ratios, and ability to interpret basic descriptive statistical reports.
  • Judgment and Situational Reasoning Ability: Ability to use functional reasoning development in performing functions within influence systems such as associated with supervising, managing, leading, teaching, directing, and controlling.
  • Ability to apply situational reasoning ability by exercising judgment, decisiveness, and creativity in situations involving the direction, control, and planning of an entire program or set of programs.
  • Physical Requirements: Ability to sit approximately 2-3 hours, stand approximately 5-6 hours, and walk approximately 4-5 hours or longer when needed.
  • Ability to operate medical equipment, stoop, squat, bend, kneel, reach, twist, climb, and balance approximately 1-33% and have hearing acuity, vision acuity far, vision acuity near, color vision, and depth perception approximately 67-100% of the time working.
  • Ability to use both right and left hands for repetitive actions such as grasping, grasping and turning, fine manipulation, speed work.
  • Ability to operate equipment and machinery requiring simple, periodic adjustments.
  • Ability to move and guide material using simple tools.
  • Ability to operate mechanical lifts, stethoscope, sphygmomanometer, and thermometers.
  • Ability to coordinate eyes, hands, feet, and limbs in performing semi-skilled movements in nursing practice.
  • Ability to exert mildly heavy physical effort in moderate to heavy work in an eight-hour workday, including lifting/carrying (0-34 lbs. approximately 34-66%; lbs. approximately 1-33%).
  • Ability to push/pull residents and other necessary items related to performing the CNA role (1-24lbs. approximately 34-66%; lbs. approximately 1-33%).
