Your expertise will ensure that patients receive comprehensive and effective care, while also collaborating with healthcare teams to achieve optimal outcomes.
Key Responsibilities- Assess and coordinate patient discharge planning needs with healthcare team members.
- Perform point-of-care testing according to established policies and procedures.
- Collaborate with physicians to develop and revise patient care plans, ensuring accurate and timely documentation.
- Coordinate and communicate with multidisciplinary teams and outside agencies to achieve expected patient care and education outcomes.
- Complete admission paperwork, verify insurance information, and conduct comprehensive medical assessments using clinical computer programs.
- Provide skilled nursing care within established plans of care, including vital sign monitoring, wound care, IV management, and physical assessments.
- Document visit notes, treatments, and care, ensuring timely, accurate, and complete documentation in adherence to regulations.
- Collaborate with patients, families, and healthcare teams to develop comprehensive plans of care based on unique patient needs.
- Graduate of an accredited school of nursing or equivalent education for licensure.
- No experience required.
