Key Responsibilities:
- Provide direct patient care as defined by the state Nurse Practice Act
- Implement plans of care initiated by registered nurses
- Document patient information accurately and timely
- Assess and educate patients and families on self-care techniques and relevant health information
- Participate in the coordination of home health services and report identified needs to registered nurses and clinical supervisors
- Use and prepare equipment and supplies effectively and efficiently, adhering to aseptic technique
- Participate in personal and professional growth and development
- Perform other duties as assigned by registered nurses
Requirements:
- Graduate of an accredited practical nurse or vocational nursing program
- Three years of nursing experience, with community/home health or medical/surgical experience preferred
- Current licensure as a licensed practical nurse or licensed vocational nurse in the state
- Valid CPR certification
- Reliable transportation, current driver's license, and appropriate automobile insurance
