We are seeking a highly skilled and compassionate RN Case Manager to join our team at UnitedHealth Group Inc. As a Care Coach Connect RN Case Manager, you will play a vital role in coordinating care and providing ongoing nursing assessment of patients and their families.
Key Responsibilities- Conduct medical home visits on established patients, performing hands-on exams and assessments.
- Provide ongoing assessments of the impact of life-limiting and/or chronic illness on patients' physical, functional, psychosocial, and environmental needs.
- Assess for caregiver burnout and provide community resources as needed.
- Implement individualized plans of care and recommend revisions as necessary.
- Perform procedures such as wound care, phlebotomy, and injections.
- Conduct advanced care discussions with patients and their families, documenting their wishes in the electronic medical record.
- Initiate preventive and rehabilitative nursing procedures as needed.
- Ensure continual assessment of patient and family needs throughout the course of care.
- Provide ongoing evaluation of patient and family/caregiver response to care, recommending alterations to the plan when necessary.
- Attend interdisciplinary meetings and contribute as a vital member of the team.
- Registered Nurse with a current license to practice in the state of employment.
- Current BLS certification or must obtain certification within 30 days of employment.
- 3+ years of experience in a physician's office, clinical, hospice, or hospital setting.
- Excellent nursing skills and ability to interact productively with individuals and multidisciplinary teams.
- Reliable transportation for daily travel to various locations as assigned.
- Hospice and Palliative Care Nursing certification.
- 2+ years of hospice experience.
- Experience with home visits, advanced care planning, and community-based nursing.
- Bilingual (English/Spanish) language proficiency.
- Proven excellent verbal and written skills, as well as organizational and prioritization skills.
