Key Responsibilities:

  • Develop and implement clinical programs and services that meet the needs of our patients and families.
  • Supervise and mentor a team of RNs, LPNs, and CNAs to ensure they have the necessary skills and knowledge to provide high-quality patient care.
  • Conduct regular home visits to ensure quality of care and identify areas for improvement.
  • Collaborate with other departments to ensure seamless care coordination and communication.
  • Develop and maintain relationships with community partners and healthcare providers to ensure access to resources and services for our patients.

Requirements:

  • Registered Nurse licensure in the state of practice.
  • Obtain and maintain active CPR certification.
  • Associates degree required.

Preferred Qualifications:

  • 3+ years RN experience in a healthcare setting (home health or hospice).
  • Medicare Skilled Nursing experience and a basic understanding of OASIS.
