Responsibilities
  • Assess patients' discharge planning needs and develop individualized plans to ensure safe and timely transitions to next level of care
  • Collaborate with interdisciplinary teams to identify and address barriers to discharge, including social determinants of health and resource gaps
  • Utilize evidence-based practices and clinical expertise to optimize patient outcomes and reduce readmissions
  • Communicate effectively with patients, families, and healthcare providers to ensure informed decision-making and coordination of care
  • Participate in quality improvement initiatives to enhance care coordination processes and patient satisfaction
Requirements
  • Graduate of accredited school of nursing or equivalent education
  • Three years' acute care experience, preferably in case management or discharge planning
  • Current RN licensure in the state of employment
  • Strong communication and interpersonal skills, with ability to work effectively in a fast-paced environment
  • Ability to analyze complex patient data and develop effective solutions
