About the Role

We are seeking a compassionate and skilled Hospice Medical Social Worker to join our team at Serenity Hospice. As a Hospice Medical Social Worker, you will play a vital role in providing emotional, social, and practical support to patients and their families during their end-of-life journey.

Key Responsibilities
  • Conduct thorough psychosocial assessments of patients and families to identify their needs and develop a plan of care.
  • Provide counseling and support to patients and families to help them cope with stress, anxiety, and other emotional challenges.
  • Collaborate with healthcare teams to ensure that patients receive comprehensive care that addresses their physical, emotional, and social needs.
  • Assist patients and families with applications for benefits and resources, such as Supplemental Security Income and Medicaid.
  • Develop and maintain relationships with community agencies and organizations to ensure that patients receive the support they need.
Requirements
  • Master's degree in Social Work (MSW) or a related field.
  • Current State licensure in Social Work (LMSW or LCSW).
  • Minimum of one year's experience in healthcare or hospice care.
  • Valid driver's license and proof of insurance.
  • Ability to pass a background check and drug test.
