Job Summary

We are seeking a highly skilled and compassionate Clinical Social Worker to join our team at Providence Home Services Oregon-Connections Palliative Care. As a key member of our interdisciplinary team, you will provide comprehensive biopsychosocial assessments, diagnoses, and interventions to patients and their support systems.

Key Responsibilities
  • Conduct thorough biopsychosocial assessments to identify patients' needs and develop personalized care plans
  • Collaborate with patients, families, and healthcare teams to implement evidence-based interventions and promote patient-centered care
  • Provide emotional support and counseling to patients and their loved ones
  • Develop and maintain relationships with community resources and healthcare providers to ensure seamless care transitions
  • Participate in interdisciplinary team meetings to share knowledge and expertise
Requirements
  • Master's degree in Social Work (MSW) from an accredited program
  • Current Oregon Clinical Social Work Associate License or Oregon Clinical Social Worker License
  • 2+ years of clinical experience in healthcare, hospice, and/or bereavement
