Key Responsibilities:

Care Coordination and Management:
  • Facilitate collaboration among the patient-centered care team to enhance care quality, promote preventive care, and minimize emergency room visits and hospital readmissions.
  • Lead a care coordination team that includes a Patient Navigator, Unit Clerk, and Medline Assistant.
  • Co-manage high-risk patients through assessments and care coordination, including necessary home and hospital visits.
  • Provide education to patients, families, and caregivers regarding the Patient-Centered Care Model.
Service Coordination and Advocacy:
  • Deliver service coordination that encompasses case management, client advocacy, and goal-oriented action plans for patients.
  • Maintain regular interactions with patients and families to ensure adherence to treatment plans, fostering self-management and shared decision-making.
  • Act as a liaison for patients concerning medical and dental benefits.
Monitoring and Reporting:
  • Oversee care delivery by documenting progress, identifying barriers to care, and maintaining communication within the Patient-Centered team.
  • Ensure compliance with HIPAA regulations to protect confidential information.
  • Collect and analyze data to enhance service activities and prepare monthly reports.
  • Additional Responsibilities: Perform other job-related duties as assigned.
Qualifications:
  • Healthcare Experience: Minimum of two years' experience working with patients and families in a community setting, public health, outreach, social work, or a related field, with progressively responsible leadership experience.
  • Supervisory Skills: Previous experience in care coordination or care management within a clinical environment.
  • Technical Proficiency: Basic data entry and computer skills are essential.
  • Interpersonal Skills: Strong ability to communicate effectively and courteously with individuals from diverse backgrounds.
  • Team Collaboration: A cooperative and industrious nature to foster positive working relationships with clinic management, physicians, clinicians, and the public.
  • Education:
    1. Graduate of an accredited registered nursing program.
    2. Current license to practice as a Registered Nurse in the Commonwealth of Pennsylvania.
Preferred Qualifications:
  • Familiarity with The Joint Commission regulations.
  • Bi-lingual capabilities in Spanish, Nepali, or French.
