Job Summary

University of Washington is seeking a skilled Nurse Care Coordinator to join our team at UW Medical Center - Montlake. As a key member of our interdisciplinary healthcare team, you will play a vital role in ensuring coordinated patient flow through an inpatient stay, culminating in a safe and timely discharge.

Key Responsibilities

Identify and monitor high medical needs patients to ensure timely completion of consults, procedures, and tests.
Facilitate coordination between providers across the continuum of care.
Lead medical team and nursing huddles to remove barriers to discharge due to medical complications and/or system or team obstacles.

Requirements

Master's degree in Nursing or related field.
Three years of professional nursing experience.
Experience in Case Management.
Ability to communicate effectively in English, both verbally and in writing.
Effective interpersonal skills, leadership skills, and clinical expertise.
