About the Role:
UW Medicine is seeking a skilled Nurse Care Coordinator to join our team at UW Medical Center - Montlake. As a key member of our interdisciplinary healthcare team, you will play a vital role in ensuring coordinated patient flow throughout an inpatient stay, culminating in a safe and timely discharge.
Key Responsibilities:
- Identify and monitor high medical needs patients to ensure timely completion of consults, procedures, and tests.
- Lead medical team and nursing huddles to remove barriers to discharge due to medical complications and/or system or team obstacles.
- Assist with ordering complex medical equipment and ensure safe transitions of care.
- Develop and monitor outcome measures related to care coordination and staff and patient satisfaction with discharge planning.
Requirements:
- Master's degree in Nursing or related field.
- Three years of professional nursing experience.
- Experience in Case Management.
- Able to communicate effectively in English, both verbally and in writing.
- Effective interpersonal skills, leadership skills, and clinical expertise.
