About the Role

Key Responsibilities
  • Assess and develop personalized care plans that address the unique needs and goals of each individual.
  • Identify and connect individuals with community resources, social services, and healthcare providers to ensure seamless care coordination.
  • Communicate effectively with stakeholders, including individuals, families, caregivers, and healthcare teams, to ensure quality and timely care.
  • Advocate for individuals and families to ensure their needs and preferences are respected and supported.
  • Conduct home visits and in-person assessments to understand the individual's needs and develop targeted care plans.
Requirements
  • Bachelor's degree in social work, rehabilitation, nursing, psychology, special education, gerontology, or a related field.
  • 2+ years of experience working in a community health setting, preferably in a healthcare role.
  • 1+ years of experience working with individuals with long-term care needs and home and community-based waivers.
  • Proficiency in MS Word, Excel, and Outlook, as well as experience with electronic charting and community resource arrangement.
Preferred Qualifications
  • Licensed Social Worker or clinical degree.
  • Background in managing populations with complex medical or behavioral needs.
