Patient Care Coordinator

We are seeking a highly skilled and compassionate Patient Care Secretary to join our team at VITAS Healthcare. As a key member of our care team, you will play a vital role in ensuring the highest level of care and support for our patients and their families.
Key Responsibilities- Screen telephone calls for the Team Manager and facilitate communication between patients/families and other team members.
- Take and maintain daily schedules for all team members, ensuring seamless coordination and collaboration.
- Attend weekly team meetings, prepare agendas, and follow through on assignments given by the Team Manager.
- Establish, maintain, and close patient charts in an organized and efficient manner.
- Provide backup documentation to the Billing Department as needed.
- Order D.M.E. and maintain records of dates ordered and picked up.
- Enter patient care and volunteer data into Vx and event tracking systems.
- Communicate with staff and outside agencies as directed by the Team Manager.
- Perform routine correspondence for the Team Manager and other team members.
- Sort and process patient care mail with other patient care secretaries.
- Maintain A.D.T. and check census daily for accuracy.
- Three years prior work experience preferred.
- Good typing skills.
- Computer literate.
- Able to handle multiple tasks.
- Excellent communication skills.
- Good telephone skills.
