Job Summary

We are seeking a highly skilled Patient Care Coordinator to join our team at Marshall Health. The successful candidate will play a critical role in supporting our physicians and healthcare providers in identifying patients who require aggressive disease management.

Key Responsibilities
  • Assist physicians and healthcare providers in identifying patients who require aggressive disease management.
  • Support primary care and sub-specialty co-management through timely communication, follow-up, and integration of disease management plans.
  • Collaborate with medical directors to measure patient outcomes and work with providers to redesign care processes and improve quality.
  • Develop and implement patient education sessions related to chronic diseases, working closely with healthcare providers.
  • Facilitate patient access to care based on individual needs and family requirements.
  • Work with the electronic medical record team to develop and manage disease-specific registries.
  • Perform other duties as assigned or requested.
Requirements
  • Education: Medical assistant or licensed practical nurse degree.
  • Experience: 3-5 years of experience in a clinical environment, with a focus on chronic disease management.
