As a Family Coach, you will work with families in a collaborative and supportive manner to help them achieve their goals. You will have a deep respect for the family's expertise and provide non-judgmental services that consider cultural diversity.

The role involves:

  • Working with families to identify strengths and utilize these strengths and build others.
  • Providing trauma-informed practices and services according to values and principles for family-centered coaching.
  • Completing intakes, monitoring family goals and activities, and providing follow-up to ensure progress or achievement.
  • Addressing housing-related issues, including lease, and providing linkages to support services such as transportation, childcare assistance, clothing, and uniform assistance.
  • Recording family progress in HMIS and case files and handling emergency and crisis situations with clients.
  • Working within contract, grant, and program requirements, including meeting timelines and reporting requirements.
  • Utilizing a team approach and working with other program staff in planning and implementation of services.

Requirements

  • A High School Diploma or equivalent is required. A Bachelor's Degree in Social Work or a related field is preferred.
  • Two years of experience in case management, including coordination of services and housing needs, is required.
  • A valid driver's license and access to a vehicle are necessary to perform essential job functions.

Compensation

The estimated salary for this position is $44,390, commensurate with years of related experience, position requirements, and candidate qualifications. About This Role

This full-time Family Coach position is based in the Calumet Team of our Permanent Supportive Housing program. The successful candidate will be highly organized, knowledgeable about Excel data tracking, and able to maintain assigned community and home-based services. Bilingual skills are preferred but not required.
