Key Responsibilities:
- Deliver direct nursing care in alignment with established protocols and policies of the healthcare organization.
- Develop and monitor individualized patient care plans.
- Act as the primary coordinator for multidisciplinary teams to ensure seamless patient care.
- Execute physician and nursing directives effectively.
- Evaluate and address the discharge planning needs of patients in collaboration with the healthcare team.
Qualifications:
- Must possess a valid Registered Nurse license from the relevant state board, obtained prior to employment.
- Credentialing from the State Board of Nursing or a current home state license for multi-state recognition is required.
- A diploma from an accredited nursing school or equivalent professional licensure is mandatory at the time of hire.
