Position Overview
LOCATION: Lancaster General Health
HOURS: Part-time, flexible based on clinic requirements, including weekend shifts
POSITION SUMMARY: To deliver nursing care to patients within a healthcare setting. This role encompasses care for patients across various age groups: pediatric (infants to 12 years), adolescent (13 to 17 years), adult (18 to 64 years), and geriatric (65 years and older).
KEY RESPONSIBILITIES: Qualified candidates must possess the capability (with or without reasonable accommodation) to execute the following tasks:
- Provides clinical guidance via phone or electronic communication based on clinical assessments and/or triage protocols, adhering to regulatory standards and best practices. Collaborates with healthcare providers as needed.
- Delivers patient-specific education covering preventive care, chronic disease management, acute conditions, medications, and other critical areas impacting patient outcomes.
- Prepares patients for consultations using established rooming standards.
- Administers medications and injections as prescribed by the healthcare provider, following departmental protocols and regulatory guidelines.
- Assists healthcare providers with various procedures, minor surgeries, and examinations as directed.
- Conducts Point of Care Testing, including but not limited to hearing, flu, urine, and strep tests.
- Collects blood and other laboratory specimens, preparing them for subsequent analysis.
- Processes prescription refills as directed by the healthcare provider.
- Supports test tracking and monitors overdue results.
- Removes sutures and changes dressings as required.
- Ensures examination rooms are well-stocked and sanitized after each patient visit. Maintains and cleans medical instruments and equipment post-procedure.
- Collaborates with providers and patients to review and adjust care plans, assessing patient progress and following up as necessary.
- Engages in proactive outreach for patients needing preventive and chronic disease services.
- Facilitates care coordination among specialists and transitions of care to gather all necessary patient information.
- Assists in coordinating referrals, education, and community resources.
- Actively participates in initiatives aimed at enhancing patient experience and quality of care.
- Engages in daily team meetings with the healthcare provider.
- Acts as a patient experience advocate, maintaining a positive demeanor in all interactions with patients, visitors, and colleagues. Communicates professionally while prioritizing customer service.
- Handles all patient information confidentially in accordance with privacy policies and procedures.
- Performs various administrative duties as needed.
- May require occasional travel; a valid driver's license is necessary if traveling.
- Participates in on-call duties as required.
- Other responsibilities as assigned.
MINIMUM REQUIRED QUALIFICATIONS:
- Completion of an LPN or RN educational program.
- Current licensure as a Registered Nurse (RN) or Licensed Practical Nurse (LPN) issued by the appropriate state board.
- AHA CPR/BLS certification required within 30 days of hire.
- This role necessitates occasional travel to multiple sites.
- One (1) to Two (2) years of nursing experience in a primary or specialty care setting.
- Prior experience in triage, emergency care, cardiac care, intensive care, or related nursing specialties.
