We are seeking a skilled and compassionate Nurse Practitioner to join our team at Lancaster General Health. As a key member of our healthcare team, you will provide high-quality patient care and support to our patients and families.
Key Responsibilities- Provide Clinical Advice: Offer expert clinical advice to patients via phone or electronic correspondence, utilizing clinical judgment and triage protocols in accordance with regulatory requirements and standards of practice.
- Patient Education: Provide personalized education to patients on preventive care, chronic disease management, acute diagnoses, medications, and other areas pertinent to patient outcomes.
- Administrative Tasks: Prepare patients for visits, administer injections and medications as prescribed, and assist providers with various procedures and exams.
- Point of Care Testing: Perform Point of Care Testing, including hearing, flu, urine, and strep tests.
- Specimen Collection: Collect blood or other laboratory specimens and prepare them for further testing.
- Prescription Refills: Authorize prescription refills as instructed by the provider.
- Test Tracking: Assist with test tracking and monitoring overdue results.
- Wound Care: Remove sutures and change dressings on wounds as necessary.
- Exam Room Preparation: Ensure exam rooms are adequately supplied and cleaned after each patient visit.
- Medical Instrument Maintenance: Maintain and clean medical instruments and equipment after each procedure.
- Care Coordination: Assist with proactive outreach for patients requiring preventive and chronic disease services, and perform care coordination activities among specialists and transitions of care.
- Referral Coordination: Assist in coordination of referrals, education, and community resources.
- Performance Improvement: Participate in performance improvement initiatives to enhance the overall patient experience and quality of care.
- Daily Huddles: Participate in daily huddles with the provider.
- Patient Advocacy: Serve as a patient experience advocate by being a positive influence at all times when interacting with patients, visitors, and colleagues.
- Education: Completion of a LPN or RN educational program.
- Licensure: Current licensure as a Registered Nurse (RN) or Licensed Practical Nurse (LPN), issued by the PA State Board of Nursing.
- Certification: AHA CPR/BLS certification required within 30 days of hire.
