Job Description:

We are seeking a highly motivated and experienced Home Visitor to join our team at the Family and Children's Agency. As a Home Visitor, you will be responsible for providing home visiting and case management services to families with children ages birth to eight years old in the Greater Norwalk Area.

Key Responsibilities:

  • Provide home visiting and case management services to a caseload of 10 to 12 families.
  • Facilitate groups and workshops as assigned.
  • Outreach to identify and recruit new clients for the MOMS Program.
  • Connect clients with appropriate referrals to community resources.
  • Accompany clients to medical and agency appointments as necessary.
  • Serve as an advocate for clients with schools, healthcare, and agency providers.

Requirements:

  • Bachelor's degree in human services, social work, early childhood education or related discipline required.
  • Experience facilitating groups preferred.
  • Bilingual - Spanish/English a plus.
  • Valid driver's license and reliable transportation required.
