Responsibilities
  • Conduct psychosocial assessments, determine patient and family needs, and develop a plan of treatment consistent with meeting those needs.
  • Act as a consultant to other disciplines, interpreting psychosocial information to patients, families, and professionals, and linking patients and their families to community support systems.
  • Provide support and crisis counseling to patients and family members, maintaining a current and complete knowledge of local, state, and federal agencies and resources.
  • Coordinate and implement discharge plans for specific patients, providing consultation services to other disciplines and completing documentation as required by Medicare and other third-party payors.
Qualifications
  • LCSW or LMSW licensure from the State of Oregon.
  • Healthcare experience preferred.
  • Masters degree from a school of social work accredited by the Council of Social Work Education.
