Core Responsibilities of LPN/RN:
Collaborates with healthcare team members to assess, plan, implement, and evaluate patient care strategies.
Administers and records prescribed medications (orally, subcutaneously, or intravenously).

Reports any adverse reactions to medications or treatments in accordance with established policies.

Maintains accurate records of patients' medical information and vital signs.
Monitors and adjusts specialized equipment used for patient care, interpreting and documenting electronic displays.
Takes corrective action when patients exhibit adverse symptoms.
Provides bedside care for a diverse range of medical patients.
Changes dressings, inserts catheters, and starts IVs.
Prepares equipment and assists physicians during patient examinations and treatments.
Responds to life-threatening situations following nursing standards and protocols.
Participates in discharge planning.
Utilizes effective time management skills.

Records all care information accurately and promptly in the appropriate formats.

Performs other related duties as assigned based on the specific setting.

Qualifications for LPN/RN:
Current LPN/RN license in the state.
Up-to-date first aid and CPR certification.
Preferred: At least one year of LPN or RN experience in a Long-Term Care environment.
Knowledge of MatrixCare is a plus.
