Case Manager-Medical Professional

As a Case Manager-Medical Professional with Human Services Administration Organization, you will provide comprehensive case management services to clients diagnosed with drug or alcohol problems. Your primary responsibility will be to ensure that these clients have access to a continuum of care by identifying, referring, and connecting them to appropriate services.
Key Responsibilities
Conduct thorough assessments of clients' needs to determine the level of care required.
Develop and implement client service plans to address their problems related to drug or alcohol usage.
Facilitate clients' treatment by coordinating access to available services, such as housing, childcare, medical, or financial services.
Monitor clients' treatment progress and update their service plans as needed.
Prepare written reports and maintain accurate client records.
Requirements
Bachelor's Degree in Chemical Dependency, Sociology, Social Welfare, Psychology, Nursing, or a related field.
One year of direct service experience performing counseling or case management work in an alcohol or other drug treatment setting.
Possess Act 33 and 34 Clearances.
Valid Pennsylvania motor vehicle license and access to a personal vehicle.
