The MDS COORDINATOR is responsible for overseeing the RAI process, ensuring that schedules are adhered to and that the MDS, CAAs, and care plans are completed in a timely manner according to federal and state regulations. This role is a full-time position reporting directly to the facility administrator.
KEY RESPONSIBILITIES and SKILLS
- Effective communication with nursing staff to facilitate the implementation of care plans.
- Leading care plan meetings with residents and their families.
- Ensuring care plans are regularly updated and accurately reflect resident needs.
- Confirming that assessments are submitted to the database with necessary amendments.
- Providing education to staff regarding ADLs and other MDS-related areas as required.
- Reviewing MDS documentation for accuracy and identifying trends that necessitate staff education.
- Monitoring Medicare and managed care documentation and providing education as needed.
- Tracking Quality Measures with a focus on those affecting the facility's five-star rating.
- Utilizing CMI for optimal outcomes and ensuring compliance with documentation guidelines.
- Understanding and implementing restorative programs.
- Engaging in the facility's Quality Improvement initiatives.
- Offering leadership in financial implications related to RUGS, PPS, and case mix rates.
- Possessing 2-4 years of experience in MDS and long-term care settings.
- Experience with Point Click Care is preferred.
- Commitment to attending seminars to stay updated with regulations.
- RN degree from an accredited institution (minimum ASN).
- Current registration in the state.
- AANAC certification is required.
- Current CPR certification is mandatory.
