Job SummaryWe are seeking a Registered Nurse - Home Health to join our team. As a Registered Nurse - Home Health, you will be responsible for providing skilled nursing care to patients in their homes. You will work closely with our interdisciplinary team to develop and implement individualized care plans that meet the unique needs of each patient.
Responsibilities- Assess patients' physical, psychological, social, and spiritual needs to develop and implement individualized care plans.
- Provide skilled nursing care to patients in their homes, including wound care, medication management, and education.
- Collaborate with our interdisciplinary team to develop and implement care plans that meet the unique needs of each patient.
- Communicate effectively with patients, families, and healthcare providers to ensure seamless care coordination.
- Document patient care and progress in a timely and accurate manner.
- Current, unrestricted state RN license.
- 1+ year experience in a clinical care setting as a nurse.
- Able and willing to travel within branch/office coverage area.
- Must have a dependable vehicle, valid driver's license, and current auto insurance in accordance with state laws.
- Must be able and willing to travel 50%.
- Associates Degree or Bachelor's Degree in Nursing is preferred.
- Previous Home Health or Hospice Experience & Oasis experience is required.
- Previous experience with HomeCare HomeBase is preferred.
